ANNUAL FEES
$2,000 annually for the first location

$1,150 annually for each additional location

 10% Discount for Renewing Members​

$250 Referral Bonus for referral of a new member

2. Please come back to this page and fill out Application Membership Data Form by clicking the "Application Data" button.

3. Pay your initial Annual Membership Fee either by Zelle, credit/debit/ACH OR by requesting an invoice via email to: jake@privatepracticenetwork.com OR pay by sending your check for the annual fee:
